Target Audience
This activity is designed for multidisciplinary healthcare professionals (HCPs) practicing in CAHs in Mississippi including specialists in vascular medicine, pulmonologists, vascular surgeons, hematologists, hospitalists, primary care providers, emergency department physicians, allied HCPs (ie, NPs, PAs, pharmacists) and/or other HCPs involved in the coordination of care between practice settings for patients with VTE.

Learning Objectives
Upon completion of this educational activity, participants should be able to:

• Discuss disparities in care among Black Americans with VTE, including disparities in receiving direct-acting oral
  anticoagulants
  
  
• Describe real-world evidence on the racial and ethnic disparities in the presentation of COVID-19, the hypercoagulability state of patients, and key practice considerations
  
  
• Implement optimal approaches to the recognition and treatment of VTE with direct-acting oral anticoagulants, including tools and resources to engage patients in their care plan
  
  
• Evaluate how a “bridging” strategy with both pre-discharge and post-discharge interventions can improve care transitions for Black patients with VTE
  
  
• Provide effective, sustainable, and culturally appropriate interventions to improve outcomes through optimal care
  transitions that reduce unplanned readmissions and health disparities for Black patients with VTE in Mississippi CAHs

Instructions for Participation and Credit

There are no fees for participating in this activity.

In order to receive credit, all participants must complete the self-assessment post-test and activity evaluation following the activity. Partial credit will not be awarded for this activity. Participants must receive a minimum score of 70% on the self-assessment to qualify for CE credit.

Certificates for CME and CNE credit may be printed immediately after successfully completing the post-test and activity evaluation.

CPE Credit will be awarded upon completion of a self-assessment and evaluation. Participation verification must be completed within 4 weeks of the activity. Your online CPE certificate will be reported directly to CPE Monitor within 4 weeks of receiving post-activity
documentation.
